Healthcare Central London’s team of Clinical Pharmacists and Pharmacy Technicians work alongside colleagues in General Practice to enhance patient care across Westminster.

What
services
does
the
Clinical
Pharmacy
team
provide?

The team provides a range of services, which include:

  • Supporting the long-term management of patients with chronic disease, such as asthma, hypertension and type 2 diabetes
  • Educating patients about their medicines and helping them to get the most benefit from their use
  • Reviewing medication for patients who are on multiple medicines
  • Providing expertise to support GPs, nurses and administrative staff with medication and prescription queries
  • Conducting medicines reconciliation reviews on transfer of care (e.g. from hospital to GP) to ensure safe prescribing
  • Implementing Care Quality Commission (CQC) searches and medication safety alerts

What
do
Clinical
Pharmacists
do?

Clinical Pharmacists provide specialist medication expertise and help to manage and review patients with long-term health conditions by optimising the patient’s medicines and signposting them to other services where appropriate.

do?

Pharmacy Technicians support patients by implementing programmes to maximise the safe and cost-effective use of medicines. This includes conducting medicines reconciliation and audits.

The team has a positive impact on both patients and Practices. This includes:

  • Helping to provide seamless, high-quality care for patients and ensuring better patient outcomes
  • Reducing patient waiting times and Practice workloads, thereby creating additional Primary Care capacity
  • Providing clinical leadership on medicines optimisation within Practices and across Primary Care Networks (PCNs)
  • Creating operational efficiencies related to medication (e.g. supporting CQC preparedness and optimising Practice revenue)

How
is
the
Clinical
Pharmacy
team
supported
by
HCL
and
its
member
Practices?

Although employed by HCL, the Clinical Pharmacy team is hosted by our member Practices and provides patient-facing care in line with the Network Contract Directed Enhanced Service (NCDES) specification and Practice requirements.

Members of the team are supported in their clinical development with supervision, education and training provided by the HCL Senior Pharmacy team. Each Clinical Pharmacist is also supported by a GP Clinical Mentor in Practice.
