ADHD Patient Portal Testing
Participant Information & Agreement
Thank you for helping us test our new ADHD patient portal.
We are asking patients to try the portal and give us feedback about how easy it is to use. Your feedback will help us identify anything that could be made clearer, easier or more accessible for future patients.
What will I be asked to do?
You will be asked to use a test version of the portal and complete some typical tasks, such as:
- Finding and completing an assessment form
- Saving an assessment and returning to it
- Booking an assessment appointment
- Sending a message to the service
A member of our team will guide the session and may ask you to explain what you are looking for or what you think something means.
We are testing the portal, not you, so there are no right or wrong answers.
The session will take approximately 45 minutes.
What information will I see?
This is a test environment. You will not see your real referral information, clinical records, diagnosis or treatment information.
You may be asked to enter information as part of the test. Please only provide the information requested by the testing team and do not enter additional personal or health information.
Your email address
We will need your email address to arrange the testing session and provide you with access to the test portal.
Your email address will be used only for purposes connected with arranging and administering the testing session and will be handled in accordance with our Privacy Notice and applicable data protection requirements.
Your ‘dummy file’ will be deleted after the test and your email address removed from the system.
Your participation
Taking part is voluntary. You can stop the testing session at any time and do not need to give a reason.
Taking part, or choosing not to take part, will not affect your healthcare, referral, assessment, diagnosis, treatment, appointments or access to our service in any way.
Your feedback will be used to improve the patient portal and patient experience. Feedback will not be used to make decisions about your individual care.
To thank you for your time, we are offering a £50 digital Amazon voucher. We will send this to you up to five working days after your testing session.
Patient testing is by invite only. You do not need an ADHD diagnosis to join the testing.
Recording
The session will be recorded to help us review the testing session. We will ask for your separate agreement before recording.

Agreement to take part
By proceeding with the testing session, I confirm that:
- I understand that this is a test of the patient portal.
- I understand that I will not see my real clinical or referral information.
- I understand that taking part is voluntary and I can stop at any time.
- I understand that taking part will not affect my healthcare or access to the service.
- I understand how my email address will be used to arrange and administer the testing session.
